根據BC省警察投訴專員辦公室（OPCC）的調查，其中一名涉案警官David Van
Patten在擔任溫哥華警方人力資源部官員時和Chan發生過不正當關係；另一名警官Greg
McCullough也與Chan發生過關係。

其中Van Patten在Chan去世一年後因行為不端被解雇，Greg McCullough被罰停職15天後自請離職。
訴訟中稱，Van Patten和Greg McCullough在Chan被聘為初級警官時，利用職權對她進行了性騷擾、性侵犯和恐嚇。

此外，警察局、董事會和工會，未能創造一種安全的工作文化和政策，放任高級警官的“應受譴責行為”持續多年。
Nicole和McCullough與2015年開始地下戀情，當時兩人都已婚，McCullough要求Chan對他們之間的關係保密。

2016年，Chan因工作關係接觸了Van Patten。此後Van
Patten一直給她發短信調情，並在一次工作旅途中強迫Chan發生性關係。
後來Van
Patten發現Chan與McCullough之前的不正當關係，並以此來威脅Chan。要求繼續與Chan發生性關係。
據Chan的家人透漏，Chan生前就飽受精神健康和抑鬱症困擾，在與兩名警官發生關係前，Chan在2012年就曾企圖自殺。
妹妹Jenn Chan表示，姐姐這種脅迫關係讓她遭受嚴重的精神困擾。
2019年1月27日，年僅31歲的Chan結束了自己的生命。
自殺前，陳於2017年曾向警察局舉報自己和兩名上司的不正當關係，但隨後被安排了壓力休假。
包括Chan的母親在內的原告們正在尋求損害賠償，包括喪失生活意義和收入支持、精神傷害和神經衝擊。聲稱“Nicole的自殺是由被告造成或促成的”。
被告方目前未提交辯護陳述。
